  • RACI, which stands for Responsible, Accountable, Consulted, and Informed.
  • It clarifies many issues that surface when working in a team environment, reduces duplication of effort, and brings contradictions, redundancies, and voids to light
  • Responsible (R): Who is responsible to complete the task.
  • Accountable (A): Who ensures the work has been done.
  • Consulted (C): Who can be consulted when there is a need.
  • Informed (I): Who needs to know what has been done.

  • The manager is mainly being informed, as the manager can’t handle all the details/tasks of the defect.
  • Engineer is responsible for identifying the root cause for the defect and design a robust solution.
  • The supervisor is accountable for containing the defect and sustain the solution.
  • While the operator is responsible for controlling the defect and sustain the solution.
